FERPA: Parent / Guardian Permission
It is recommended that you obtain parent permission prior to contacting the health plan. Policies regarding documentation of permission may vary by school district. Some may allow documentation of verbal consent; others may require a signature.
FERPA and HIPAA were identified as barriers to making successful linkages between school nurses and health plans. In 2017, a partnership led by the Missouri Asthma Prevention and Control Program and Missouri School Boards Association developed a FERPA-compliant form to obtain consent for sharing information contained in student records with the student’s health care provider and health plan. A Word version is made available below so it can be modified by schools, as needed.
It is recommended schools distribute this unified form during the student registration process. View the Power Point "Why Upgrade the FERPA Form" for a description of the benefits of using a new FERPA form.
